Can Semaglutide or Tirzepatide Affect My Eyesight?

Clinic patient education · Evidence checked 3 October 2026

Learn about Dr. Fowad Shahzad’s clinical focus and professional background.

Semaglutide and tirzepatide can raise questions about blurred vision and, more rarely, serious eye problems. These concerns need careful separation: temporary changes in focusing, diabetic retinopathy and damage to the optic nerve are different conditions. Current evidence does not justify saying that tirzepatide is free of eye risk or that semaglutide causes more of every kind of vision problem.

Sudden vision loss needs urgent assessment

Seek emergency eye assessment immediately for sudden loss of vision, including partial loss, or rapidly worsening eyesight. Do not wait for a routine weight-management appointment or assume it is an expected treatment effect. The UK medicines regulator gives this advice for semaglutide, and Australia’s regulator advises urgent assessment across these medicines. Take the medicine name and dose information with you. MHRA safety advice.

Temporary blurred vision is different from eye damage

Changing blood glucose can affect fluid levels in the eye’s focusing tissues. The US National Institute of Diabetes and Digestive and Kidney Diseases explains that blurred vision may occur for days or weeks during changes to diabetes treatment and can settle as glucose approaches normal. This explanation cannot diagnose a new symptom or rule out a more serious problem. NIDDK: diabetic eye disease.

Diabetic retinopathy involves the retina

Diabetic retinopathy damages blood vessels in the retina. Rapid improvement in glucose control can temporarily worsen existing retinopathy. Both medicines’ current US prescribing information advise monitoring people with a history of this condition. Ozempic prescribing information, section 5.3; Mounjaro prescribing information, section 5.7.

In a two-year trial involving people with type 2 diabetes and high cardiovascular risk, retinopathy complications occurred in 3.0% receiving semaglutide versus 1.8% receiving placebo: approximately 30 versus 18 per 1,000 participants. These were retinopathy complications, not a count of blindness or NAION. They are also not a semaglutide-versus-tirzepatide comparison.

NAION involves the optic nerve

Non-arteritic anterior ischaemic optic neuropathy, or NAION, concerns reduced blood supply to the optic nerve and can cause lasting visual loss. In June 2025, the European Medicines Agency recognised it as a very rare semaglutide side effect. Its review estimated approximately one additional case per 10,000 person-years of treatment in adults with type 2 diabetes. This is an estimated excess over the comparison group’s risk, not a prediction for every patient. EMA review.

Australia extended precautionary product warnings across the class in July 2026, explicitly including tirzepatide. The regulator distinguished stronger evidence supporting the semaglutide signal from less conclusive evidence for other drugs. This does not establish that all medicines have equal risk. TGA safety update.

Is one medicine safer for the eyes?

A 2025 observational study directly compared 3,806 matched patients per drug and recorded two NAION cases with semaglutide and one with tirzepatide. The researchers said the small number prevented statistical significance testing. These figures cannot establish a reliable safety ranking. Wang and colleagues, JAMA Network Open.

Discuss previous retinopathy, other eye disease and recent glucose changes with your prescriber. Keep recommended eye follow-up. Sudden symptoms need urgent assessment; if NAION is confirmed during semaglutide treatment, regulators advise stopping that medicine under the treating team’s direction. Routine decisions about continuing or switching treatment require an individual benefit-and-risk assessment.
