How Much Weight Loss Can I Expect with Mounjaro per Month?

Clinic patient education · Evidence checked 3 October 2026

Learn about Dr. Fowad Shahzad’s clinical focus and professional background.

There is no reliable fixed number of kilograms that everyone should lose each month on Mounjaro (tirzepatide). Progress varies, and the first month is not a fair test of the final result. A useful answer explains the treatment stage, percentage of starting weight, health changes and the exact product being used.

This patient guide addresses a frequent question in obesity consultations: does a good-quality imported product guarantee faster weight loss? It does not. Authenticity is essential for safety, but it cannot guarantee an individual response.

What do the major trials actually show?

In SURMOUNT-1, 2,539 adults with obesity, or overweight with a weight-related complication, without diabetes were studied for 72 weeks. Average weight reductions across the three tirzepatide groups were 15.0%, 19.5% and 20.9%, compared with 3.1% with placebo. The study included gradual dose escalation and lifestyle support. These are cumulative group averages over about 16.5 months, not monthly targets or guaranteed results. Read the original SURMOUNT-1 trial.

For illustration only, 15% of a starting weight of 100 kg is 15 kg. The same percentage at 80 kg is 12 kg. This arithmetic explains why comparing kilograms alone can mislead; it does not predict either person’s result. Dividing a 72-week trial average into equal monthly portions would hide the changing pace of treatment.

SURMOUNT-5 directly compared tirzepatide with semaglutide in 751 adults with obesity without diabetes. At 72 weeks, average reductions were 20.2% and 13.7%, respectively. This open-label, manufacturer-funded trial supports greater average weight reduction with tirzepatide in that population. It does not establish a monthly guarantee, universal safety superiority or equivalent performance for every alternative brand. Read the SURMOUNT-5 results and design.

How should progress be assessed month by month?

  • The first month: review tolerability, nutrition and hydration.
  • During escalation: assess the weight trend alongside appetite, food intake, strength and side effects. Escalation is a clinical decision, not a race to a higher dose.
  • Later treatment: review sustained change and maintenance needs. A slower phase or plateau needs interpretation in context, rather than an automatic dose increase.

The U.S. weight-management label specifies gradual escalation, at least four weeks between increases, and review of response and tolerability. Local prescribing must follow the exact authorized product information in Pakistan. See Zepbound prescribing information, sections 2 and 14.

Does an imported alternative work exactly like Mounjaro?

Mounjaro is a brand of tirzepatide. A different manufacturer’s product is a separate product. An imported label, familiar-looking box or claim to contain the same ingredient does not by itself establish equivalent quality, device, formulation, storage or clinical evidence. Do not assume the original trials tested a particular alternative brand.

Ask a licensed pharmacist and prescriber to verify the exact manufacturer, product, strength, batch, expiry, source, storage history and current registration or lawful supply basis. DRAP has warned about falsified GLP-1 products. Its older alert is evidence of this risk, not a complete statement of today’s registrations. DRAP product-safety alert; current DRAP product register.

What should I bring to a progress review?

Bring your weight record, medicine packaging, dose history and notes about meals or side effects. Seek prompt care for persistent vomiting, inability to keep fluids down or severe ongoing abdominal pain. The product information describes these safety concerns.

The aim is a sustainable health improvement with adequate nutrition and physical function. A fixed promise such as “5 kg every month” is not an evidence-based way to judge treatment.
